AMC has relinquished all Student Academic Records to the Florida Department of Education Commission for Independent Education (CIE). If you need an official transcript, you can call 1-888-224-6684 toll-free or complete a transcript request form at https://web02.fldoe.org/CIE/Transcript/TranscriptRequest.aspx. You may also send an email request to: cieinfo@fldoe.org.

- IRS Form 1098-T: Tuition Statement forms report tuition and fee payments made by students that qualify for certain educational tax credits. It contains information to assist the Internal Revenue Service (IRS) and you in determining if you are eligible to claim educational related tax credits. Acupuncture and Massage College (AMC) is required to file this form with the IRS for all enrolled students for whom a reportable transaction is made during the 2024 calendar year. If you were eligible for a 1098-T, your form will be mailed via USPS to the address the college had on record for you. Forms will be mailed by January 31, 2025.
Instructions for Licensure Applications
Massage Therapy License:
- Go to the MBLEx Link to register for Exam: https://fsmtb.org/
- Once you pass your MBLEx, then you need to go get your fingerprints done with a Livescan service providers that is approved by the Florida Department of Law Enforcement. Below is a link to where you can find one near your area. They require an ORI number, which is: EDOH4600Z
- After getting your fingerprints scanned, you can then apply with the Board of Massage Therapy via this link:

Important Information
Step-by-step instructions for:
- NCCAOM Certification
- FL Board of Acupuncture Licensing
You will NOT be authorized to take your “Board” exams unless you have completed and received your Clean Needle Technique Certification from the CCAHM.
STEP 1: NCCAOM for EXAMS
Create an account with NCCAOM on their Certification Management Portal.
- Go to: Login AS5032 NCCAOMCERT (cyzap.net)
- Request that your AMC transcript be sent to NCCAOM.
- This may be done by emailing the FL CIE at cieinfo@fldoe.org.
STEP 2: NCCAOM & CNT certification from CCAHM
- Request CNT Certificate Sent to NCCAOM (You can do this the minute you have an NCCAOM account)
- Go to CCAHM and register your email and create an account. If your CNT certificate has expired, call the CCAHM at (410) 464-6040 to re-certify.
- Go to: https://www.ccahm.org/ccaom/Make_a_Request.asp
- Follow the steps to “register”. Then, if you do not receive an email to set your password, CALL THEM to resolve. The tel. # is above.
- It will be free of charge.
- Be sure to reference your NCCAOM candidate ID # if there is a place to enter it, so the verification goes directly to your NCCAOM account and file.
- Once Step 1 and Step 2 are completed, you will receive an AUTHORIZATION TO TEST LETTER (ATTL) from the NCCAOM with instructions on creating an account with Pearson Vue where you can schedule and pay for the exams. Each of the 4 exams is $325.
STEP 3: LICENSE APPLICATION (after passing 4 exams)
- AT THE APPROPRIATE TIME, apply for your license online with the FL Board of Acupuncture by registering at you MQA Portal:
- You may want to wait until you pass all 4 NCCAOM exams first.
CHOOSE APPLICATION BY EXAMINATION
- You may want to enter the minimum liability on the application (of course you should obtain it when you receive your license and begin treating patients)
STEP 4: LICENSE APPLICATION (60 credits transcript)
- IMMEDIATELY after, or just before, applying for your license, request Official Transcripts verifying proof of 60 college credits from your previous school that were the prerequisite for the OM program at AMC. This is the transcript you had sent to the Admissions Department when you enrolled at AMC. But you must request those transcripts FROM THE PREVIOUS INSTITUTION. It cannot come from AMC or from you. Be aware that this may take longer if you attended schools outside of the United States.

STEP 5: LICENSE APPLICATION (verification from NCCAOM)
- AFTER YOU HAVE PASSED ALL EXAMS and AFTER NCCAOM has received your CNT Certificate and AFTER having submitted your license application, go to your NCCAOM Portal request the NCCAOM Exam Verification State Notification.
- NCCAOM will send your Certified Diplomat Status and Exam scores to the FL Board of Acupuncture.
- DO NOT request this until you have already applied for your License with the Florida Department of Health (Board of Acupuncture)
- DO NOT request this until NCCAOM has received your CNT Certificate and until the NCCAOM Portal shows that you have passed all 4 exams. (It takes several days for them to receive the results from Pearson Vue.)

STEP 6: Contact CIE for Transcript to be sent to the Board
- All transcript requests must be emailed to cieinfo@fldoe.org
- Request that your AMC transcript be sent to the FL Board of Acupuncture. All AMC Graduate Transcripts must be the final official school copy and submitted directly from FL CIE.
STEP 7:
- ONLY if the Acupuncture Injection Therapy (AIT) course was NOT a part of your AMC program and is not on your AMC Transcript.
- If you have taken the 60-hour AIT course and have an AIC Certificate, you must mail a copy of the certificate to the Florida Board Acupuncture. Please write your File number on the copy of the certificate, so they do not misplace it at the FL Board offices. To avoid delays with this, you should also upload a copy to your Department of Health-MQA Dashboard and fax them a copy to FAX: 850-617-6460.
